- Marissa was obtaining scheduled ibuprofen 600mg from Pyxis. Marissa used our SAEFTY skill STAR - Stop, Think, Act, Review and identified ibuprofen 800mg was dispensed instead of 600mg. It was then identified all the ibuprofen in the cubie was 800mg not 600mg. Thank you, Marissa, for preventing a safety event by focusing on the task!
- Andrea entered operating room to prepare for next surgical case and identified the room was not fully cleaned and some biohazard items still in room. Andrea used our SAFETY skill ARCC – Ask a Question, Request a Change, Voice a Concern and Use Chain of Command for immediate resolution followed by another SAFETY skill iSAFE – electronic reporting system. - Stephenie received a medication order which was verified and dispensed. Upon receipt, Stephenie used our SAFETY skill Reference Use and identified the medication received is usually given orally and the wrong medication was sent. - Edison completed a scan for CT Abdomen Pelvis when he noticed on imaging there were no lung markings on patient’s right lung. After completion of another scan, Edison used our SAFETY skill QVV - Quality, Validate, Verify and contacted the Emergency Department physician to verify suspicions of a pneumothorax. Additional imaging was ordered by physician and pneumothorax was confirmed. Thank you, Edison, for preventing a safety event by embracing a questioning attitude!
- Melani received handoff from CNA regarding patient’s unsteadiness getting to bathroom. Melani used out SAFETY skill QVV - Quality, Validate, Verify to assess the patient further and identified patient’s condition had changed from previous day and called a stroke alert. Thank you, Melani, for preventing a safety event by embracing a questioning attitude!
- Txhuj was checking on patients on unit when an alarm sounded. Txhuj used our SAFETY skill Reference Use and responded to alarm. Upon arrival to room, it was noted the patient's oxygen saturation was low at which time Txhuj escalated for assistance. Thank you, Txhuj, for preventing a safety event by taking thoughtful action.
- Joash questioned why a patient with a history of a multi-drug resistant organism was not on isolation. Joash spoke with infection control to clarify precautions then escalated implementation. Joash used our SAFETY skill ARCC – Ask a Question, Request a Change, Voice a Concern and Use Chain of Command to address appropriate isolation precautions. - Nepher noted a post-acute covid patient was not on isolation even though the patient tested positive in ED, was symptomatic, and was receiving bipap treatment.
